Learn the Pros and Cons of an Integrated Plan Versus Individual Plans

Consider the following before choosing whether to create an integrated Plan or create individual Plans for the selected Youth:

By including the caregiver assessment with the youth assessment in an integrated plan, the caregiver plan will be marked "Completed" and will need to be Amended to make changes.

Updating the caregiver information on the youth plan will not update the caregiver plan.

Future updates to the caregiver plan will not be reflected in the Integrated Plan.

Details related to the caregiver plan will be excluded from Aggregate Reports and the Supervisor Dashboard.

If the youth has multiple caregivers, the caregiver assessment that was done closest to the youth's will be selected--it can be done either before or after, however it must be within two weeks of the youth assessment.

If the caregiver assessment was done after the youth assessment and the youth assessment was marked "Completed", it must be Amended to create an integrated plan.
